This guide provides some basic information about the effects ostomy surgery has on digestion and food absorption. After healing is complete and the ostomy is functioning normally, most people with ostomies can return to a normal diet. If you have a special diet because of heart disease, diabetes or other health problems, you should ask your doctor about a diet that will work with both that problem and your ostomy. More detailed information about any special dietary and nutritional needs can be obtained through a registered dietitian (nutrition specialist).

Overview of Common Ostomy Diversions
The following is an overview of common ostomy diversions that will help you better understand their effect on the digestive system. Both fecal and urinary diversions interrupt the normal process of elimination and will have some influence on its function.

Bowel Diversions
Colostomy: A portion of the large intestine is removed or by-passed. The remaining portion of the functioning large intestine (colon) is brought through the abdominal wall, creating a stoma.
Ileostomy: The entire colon, rectum, and anus are removed or by-passed. The small intestine (ileum) is brought through the abdominal wall, creating a stoma. Continent Ileostomy (abdominal pouch): An internal reservoir with a nipple valve is constructed from a section of small intestine and the valve is brought through the abdominal wall to create a stoma for intubation to empty the reservoir/pouch.
Ileoanal Reservoir/Pelvic Pouch: A pelvic pouch/reservoir is constructed from small intestine, which is brought down and attached to the anus. Evacuation is through the anus.

Sources and Metabolism of Vitamin D

Humans get vitamin D from exposure to sunlight, from their diet, and from dietary supplements.1-4 A diet high in oily fish prevents vitamin D deficiency.3 Solar ultraviolet B radiation (wavelength, 290 to 315 nm) penetrates the skin and converts 7-dehydrocholesterol to previtamin D3, which is rapidly converted to vitamin D3 (Fig. 1).1 Because any excess previtamin D3 or vitamin D3 is destroyed by sunlight (Fig. 1), excessive exposure to sunlight does not cause vitamin D3 intoxication.2 Few foods naturally contain or are fortified with vitamin D. The “D” represents D2 or D3 (Fig. 1). Vitamin D2 is manufactured through the ultraviolet irradiation of ergosterol from yeast, and vitamin D3 through the ultraviolet irradiation of 7-dehydrocholesterol from lanolin. Both are used in over-the-counter vitamin D supplements, but the form available by prescription in the United States is vitamin D2.

Vitamin D from the skin and diet is metabolized in the liver to 25-hydroxyvitamin D (Fig. 1), which is used to determine a patient’s vitamin D status1-4; 25-hydroxyvitamin D is metabolized in the kidneys by the enzyme 25-hydroxyvitamin D-1á-hydroxylase (CYP27B1) to its active form, 1,25-dihydroxyvitamin D.1-4 The renal production of 1,25-dihydroxyvitamin D is tightly regulated by plasma parathyroid hormone levels and serum calcium and phosphorus levels.1-4 Fibroblast growth factor 23, secreted from the bone, causes the sodium–phosphate cotransporter to be internalized by the cells of the kidney and small intestine and also suppresses 1,25-dihydroxyvitamin D synthesis.5 The efficiency of the absorption of renal calcium and of intestinal calcium and phosphorus is increased in the presence of 1,25-dihydroxyvitamin D (Fig. 1).2,3,6 It also induces the expression of the enzyme 25-hydroxyvitamin D-24-hydroxylase (CYP24), which catabolizes both 25-hydroxyvitamin D and 1,25-dihydroxyvitamin D into biologically inactive, water-soluble calcitroic acid.2-4

Endorphins are substances produced in the brain that naturally raise the pain threshold. Some of us produce more endorphins than others, and those that produce very little are highly sensitive to pain. Another way of describing the phenomenon of pain sensitivity is in the recognition that some of us produce more of the enzyme that blocks the utilization of our own endorphins, thus making us more sensitive to pain. Bis beta-carboxyethyl germanium sesquioxide inhibits the endorphin inhbitor.

Personal use, and reports from physicians and patients, indicate that Germanium does work as a pain desensitizer. In both humans and animals Ge132 has been shown to increase gamma interferon in the blood, activate macrophages and natural killer cells, bring blood hemoglobin levels up and white cell counts down, stimulate immunomodulation activity in the B cell system, and demonstrates antitumor and antiviral activities. This substance, therefore, may be an excellent adjuvant (aids the operation of) immunochemotherapeutic agent. The effects of Ge132 on various immune parameters are almost identical to that of known gamma interferon immunomodulating activity. In addition, studies on immune-suppressed animals and on patients with malignancies or rheumatoid arthritis suggest that Ge132 normalized the function of T cells, B lymphocytes, anti-body-dependent cellular cytotoxicity, natural killer cell activity and numbers of antibody-forming cells. Obviously organic germanium has a “normalizing” influence on the immune system.

A key characteristic of Germanium is its ability to “normalize” many physiological functions. For example, it has the ability to lower blood pressure on hypertensive people. This normalizing effect may extend to a broad variety of physiological/biochemical functions, such as restoration of deviant blood parameters to their normal ranges, including pH, potassium, calcium, chloride, triglycerides, cholesterol, bilirubin, uric acid, and blood hemoglobin.

Teens whose parents are warm and firm achieve more in school and report less depression and anxiety.

When mothers discussed condom use before teens initiated sexual intercourse, youth were three times more likely to use condoms after becoming sexually active.

Teens who reported previous discussions of sexuality with parents were seven times more likely to feel able to communicate with a partner about HIV/AIDS.

When parents make consistent efforts to know their teen’s friends and whereabouts, young people report fewer sexual partners, less sex and more use of birth control.

Researchers have not found that sex education increases the risk of early sexual activity.

Discussing HIV with parents decreases the likelihood that teens will engage in unprotected sexual intercourse.

DO WE KNOW THEY WILL BE FINE?
But is it true? Research tells us no such thing, simply because there is no reliable body of research that compares children being raised in same-sex versus mother/father homes. We are just beginning the same-sex family experiment; therefore there are not significant populations of such families for scientists to observe over long periods of time.
Honest researchers confess as much:

“Research exploring the diversity of parental relationships among gay and lesbian parents is just beginning.”4

“Thus far, no work has compared children’s long-term achievement in education, occupation, income and other domains of life”5 (emphasis added).

From a sound methodological perspective, the results of these studies can be relied on for one purpose – to indicate that further research…is warranted. The only acceptable conclusion at this point is that the literature on this topic does not constitute a solid body of scientific evidence.”6

Studies on same-sex parenting are plagued with “persistent limitatation[s]. …As a result, we cannot be confident concerning the generalizability of many of the findings. …”7

The authors of an American Sociological Review study, though personally sympathetic with the idea of the same-sex family, “disagree with those who claim that there are no differences between the children of heterosexual parents and children of lesbigay parents. …”8

Worried About Your Child’s Weight?

The following are 10 physician-approved tips for parents:

Discuss the importance of healthy diet and lifestyle choices with your child. Remember to talk about health, not about weight.

Create a healthy eating plan; do not put your child on a diet. Many diets that are popular with adults have never been tested on children, and a healthy, balanced diet is the goal for children.

Allow sweets or treats, such as candy and other snacks, in moderation rather than eliminate them. Children who learn the importance of balanced eating will be better-equipped to manage their own diet as adults.

Give your child plenty of healthy alternatives. Focusing only on what he or she cannot eat does not teach your child to make healthy choices.

Do not use food as leverage in a relationship. Do not hold back food as a punishment, and do not offer food as a reward.

Encourage your child to make better food choices. Communicating your interest in his or her health is more effective than focusing on shortcomings.

Have your child sit down, eat a balanced meal, and then move on to a fun activity. Do not let your child eat in his or her bedroom or in front of the television, constantly leaving and returning to the table.

Practice what you preach. If you eat right, it will be easier to get your child to eat right as well.

Be a role model. Be active yourself.

Promote better nutrition by having regular meal and snack times. Restrict all-day snacking.

The primary principle when learning photography is to take many pictures. Never take just one. When you think a few is enough, take a
dozen. Traditional photography requires a wait between the taking of a picture and the development of the film for viewing. With digital photography, the ability to immediately view and edit images gives us the freedom to make mistakes and to quickly learn from them. For this reason, digital photography supports the primary requirement for mastering photography, which is to take many pictures.

Another advantage of digital photography is its relatively low cost when compared to traditional photography. Traditional film based photographs had to be processed or sent to the lab which could take days and incur substantial film and processing costs. Polaroid photography offers immediacy but the film is costly so it is not an acceptable alternative for the average photographer. By comparison, digital photo media can be used and reused endlessly which significantly cuts down on the cost. Cost control is the second advantage of digital photography.

Some people are perfectly happy taking pictures and living with what the lab sends back. But there are other photographers who want to. have more control over the processes that produces the final image.

CONTROLS: FRONT of CAMERA
Focus Mode (the little lever at the bottom of the lens marked AF and M [hidden above]): I set it to AF. This means Auto Focus. M means manual focus. In manual focus you have to twist the focus ring yourself and look for a sharp image in the viewfinder. In manual you also can look for the green dot at the bottom left in the viewfinder. The green dot lights up when you’re in focus.

Depth-of-Field Preview (the little button below the lens in the photo above): Tap this to stop the diaphragm down to the taking aperture. The viewfinder probably gets darker, but look carefully and you can see what’s in focus or not. This analog feature is a remnant from film days. Today most people look at the LCD playback.

Flash Bolt Button (left side of flash hump, as seen from the rear): This does several things depending on how and when you press it.
1.) If the flash is down, press the flash button and the flash pops up.
2a.) If the flash is up, press and hold the flash button and turn the front dial to change the flash exposure compensation. This sets the brightness of the flash. + makes the flash brighter, - makes it dimmer. This setting only changes the brightness of the flash. It leaves the background ambient exposure alone. Set it to - if your subjects are getting washed out. If you run out of flash power beyond 10 to 20 feet then setting it to + can’t make the flash any brighter.
If you set flash exposure compensation to anything other than zero you’ll see a little “+/- bolt” icon in the finder and on the top LCD.
2b.) If the flash is up, press and hold the flash button and turn the rear dial to change the flash sync mode. You’ll see it on the top LCD in the box with the bolt.

This document explains digital terminology for the professional photographer who is new to computers and digital photography.

Aperture is a powerful digital photography application designed to help you produce the best images possible. However, many factors outside of Aperture can affect the quality of your images. Being mindful of all these factors can help prevent undesirable results.

The following chapters explain how your camera captures a digital image, how images are displayed onscreen and in print, and how cameras, displays, and printers measure image resolution.

How Digital Cameras Capture Images
If you’ve previously shot film and are new to digital media, this chapter is for you. Here you’ll find basic information about the types of digital cameras, camera components and concepts, and shooting tips.

People take photographs for many different reasons. Some take pictures for scientific purposes, some shoot to document the world for the media, some make their living shooting products for advertisements, and others shoot for enjoyment or purely artistic purposes. Whatever your reason for picking up a camera and framing an image, an understanding of how cameras work can help you improve the quality of your images.

In reality, there is little if any reason to convert this document into Adobe RGB (1998). sRGB is a smaller gamut color space than Adobe RGB (1998) so converting on opening the document didn’t make the color gamut of this document any larger. For whatever reason, the original user of the Dog_in_Bowl.tif document wanted it in sRGB. So preserving the original profile as we did the first time we opened the document makes sense. If the Dog_in_Bowl.tif document is open, close this image and if asked to save, click Don’t Save or just hold down the D key. Go back into the Color Settings once again. This time set the policy for RGB documents to Off and click OK.

1. Now reopen the Dog_in_Bowl.tif document once again. The Embedded Profile Mismatch opens yet a third time. The RGB working space is still set to Adobe RGB (1998) and the Dog_in_Bowl.tif document is still not in that color space. Notice that this time, the radio button defaults to Discard the embedded profile (don’t color manage) as seen in Fig. 9-5-5. Click OK to open the document

If you examine the document’s profile indicator at the bottom of the document window, you will see it now says Untagged RGB. The preview of the image shouldn’t look correct either! This is because Photoshop will use the currently selected working space, in this case Adobe RGB (1998), as a mechanism for previewing documents, which are untagged. The Dog_in_Bowl.tif document is in sRGB. However, since we discarded the color profile, it opens untagged. Photoshop therefore uses Adobe RGB (1998) (the color space set in the Color Settings) as the assumption for previewing this document.
